Constellation Health Services is a leading provider of hospice care, dedicated to delivering compassionate and dignified end-of-life care to patients and their families. We are seeking an experienced Licensed Practical Nurse (LPN) to join our clinical team as an End of Life Care Specialist.

Job Summary

As an LPN at Constellation Health Services, you will play a critical role in providing high-quality patient care and collaborating with our interdisciplinary team to ensure positive outcomes. Your responsibilities will include:

  • Delivering nursing care in accordance with the State Nurse Practice Act, with a focus on end-of-life care.
  • Implementing current nursing practices based on comprehensive assessments and care plans.
  • Maintaining accurate and timely documentation of patient services.
  • Providing patient and family/caregiver education and support.
  • Recognizing and addressing changes in a patient's condition.
Requirements

To be successful in this role, you will need:

  • A graduate degree from an accredited practical nurse program.
  • Connecticut State Licensed LPN with a minimum of 1 year of clinical experience.
  • Preferably, experience in hospice, palliative or community health home care or medical/surgical settings.
