Job Summary

This is an exciting opportunity for a Statewide Travel Palliative Care Nurse Practitioner to join our Advanced Illness Management team. As a key member of our team, you will provide advanced illness management and palliative care services to patients across the state.

Salary and Benefits

The estimated salary for this position is $96,390-$120,490 per year, with comprehensive benefits including health insurance, 401k plan, tuition reimbursement, and paid time off.

Responsibilities

You will be responsible for:

  • Facilitating and providing advanced illness management and palliative care services to patients in their homes
  • Developing individualized treatment plans and providing comprehensive care under the direction of the Palliative Medical Director or Primary Care physician
  • Traveling extensively throughout the state to provide patient care and other essential responsibilities
Requirements

To be considered for this role, you must have:

  • A Master of Science in Nursing from an accredited school of nursing or equivalent as allowed under state law
  • At least 2 years of experience as a Nurse Practitioner
  • An active and unrestricted Nurse Practitioner certification/license
  • Multi-state licensure desirable or willingness to obtain additional state licensure
Preferred Qualifications

Certification in Hospice and Palliative Care (ACHPN), Nurse Practitioner specialty in Family or Adult Medicine, Acute Care, and federal DEA license are preferred qualifications.
