Hospice Social Worker II - Full-Time Day Position

About the Role

This full-time day position is part of our North Regional Team, working Monday through Friday. With an average patient caseload of 30-35 patients, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.



Responsibilities

As a Hospice Social Worker II, your primary responsibilities will include:



  • Assessing the psychosocial needs of patients and their families to develop effective and comprehensive plans for psychosocial interventions and complex discharges
  • Collaborating with the multidisciplinary team to ensure effective, efficient, and appropriate care
  • Providing education and expertise to promote the practice and profession of social work within the hospital
  • Serving on specialty areas within the hospital that require a MSW for regulatory or grant funding requirements


Requirements

To be successful in this role, you will need:



  • A Master's Degree in Social Science or Social Work
  • BLS/CPR (Basic Life Support for Healthcare Providers)
  • Licensed Social Worker
  • Computer literacy
  • Experience in grief and bereavement counseling, alcohol and drug intervention, and support for victims of abuse, neglect, and violence


Estimated Salary: $60,000 - $80,000 per annum
