Job Summary:

">

The Case Manager I will provide case management services, assisting patients in obtaining income, insurance, and housing. This includes assistance with co-occurring disorders (mental health and substance abuse) to improve their current functioning level and skills acquisition through assessment, engagement, access to treatment, rehabilitation, and goal setting.

Responsibilities:

">
  • ">
  • Meet with each patient individually and conduct an intake needs assessment, documenting progress notes and completing service delivery logs.">
  • Manage a minimum of 50 patient caseloads.">
  • Write and enter progress notes daily into Electronic Health Record (EHR) systems to maintain patient files.">
  • Perform duties primarily in-office and traveling to conduct in-person member visits.">
  • Manage work schedules when meeting with members.">
  • Transport patients to schedule appointments and community resources as needed.">
  • Participate in care coordination meetings with various partners as deemed necessary.">
  • Represent and advocate for patients in obtaining identification, housing, benefits services, and entitlements from governmental and private agencies and organizations.">
  • Coordinate linkage to community resource programs and supports.">
  • Collaborate with patients in developing individualized SMART treatment plans.">
  • Rotate after-hours on-call phone duties for a week at a time, including weekends.">
  • Participate and complete job-related training, certification, and renewal requirements.">
  • Be flexible with work location, tasks may vary based on program needs.">
  • Attend regular meetings with management and team.">

Requirements:

To be successful in this role, you will need:

  • A high school diploma or GED with relevant life experience, or an Associate's degree in social services preferred.">
  • Excellent communication and interpersonal skills, both verbally and in writing.">
  • A commitment to providing quality care to underserved populations.">
  • The ability to work collaboratively in a team environment, respecting diverse perspectives and experiences.">
  • Maintain appropriate boundaries among patients, peers, and WellSpace partners/stakeholders.">
  • Demonstrate adaptability in a fast-paced environment.">
  • Familiarity with Microsoft Word, Excel, and Outlook email software.">
  • A valid driver's license with an insurable driving record.">
  • A willingness to transport patients using personal vehicles.">

Work Environment:

This position involves working in a typical office environment, frequently requiring sitting, using hands and arms to reach, standing, walking, reaching with hands and arms, occasionally lifting and/or moving up to 20 pounds. The employee must also possess hearing and speech to communicate in person and over the phone.
