Physical Therapist Assistant

We are seeking a highly skilled Physical Therapist Assistant to join our team at All Medical Personnel. As a key member of our interdisciplinary team, you will provide holistic care to patients in the comfort of their own homes.
About the RoleThis is an exciting opportunity to work as a Physical Therapist Assistant in home health settings, providing physical therapy services to pediatric, adolescent, adult, and geriatric patients. You will be responsible for collaborating with the patient/family/caregiver and primary care provider to develop a personalized plan of care focused on achieving the best possible outcomes.
Main Responsibilities- Collaborate with the interdisciplinary team to develop a patient-centered plan of care
- Provide skilled, intermittent physical therapy services in the patient's home
- Instruct, coach, guide, and intervene to support patients and families in achieving their physical therapy goals
- Practice independently in the patient's home using evidence-based practice for assessment, clinical decision making, and coordination of care across the continuum
- Act as case manager and team lead for patients requiring home health therapy services
- Assess patients and collaborate with the interdisciplinary team and primary care provider to develop the patient-centered plan of care
To be successful in this role, you should have:
- 1-2 years of experience in an outpatient rehabilitation setting
- At least two (2) years of physical therapy experience highly preferred
- One (1) year experience in home health or other health setting with geriatric emphasis
- Current license in Physical Therapy or ability to obtain prior to hire
- American Heart Association HealthCare Provider BLS certification required within 90 days of hire
- Excellent customer service skills
- Ability to work without direct supervision
- Knowledge of physical therapy assessment and treatment methods
We offer a competitive salary of $$$76,000-$96,000 per year based on experience. This is a full-time position with opportunities for professional growth and development.
