Care Coordinator

Key Responsibilities:
Develop and implement personalized care plans that cater to the unique needs of each client
Conduct thorough assessments to identify client goals and objectives
Collaborate with healthcare professionals to ensure seamless care coordination
Maintain accurate and up-to-date records of client progress
Requirements:
Current and valid license to practice as a Registered Nurse in the state of Texas or a Master Social Worker (LMSW) in the state of Texas
Certification in Case Management required within two (2) years of hire into the Case Manager position
