Job Summary:

We are seeking a skilled LPN Staff Coordinator to join our team at MMLiving. As a key member of our clinical staff, you will play a vital role in providing exceptional patient care and supporting the development of our community programs.

Key Responsibilities:

  • Collaborate with healthcare professionals to identify patient needs and develop effective care plans.
  • Assist in the direction of programs and ensure compliance with local, state, and federal laws.
  • Establish and maintain strong relationships with physicians, community agencies, and other healthcare professionals.
  • Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.
  • Provide effective communication to patient/family, team members, and other health care professionals.

Requirements:

  • Graduate of an accredited school of vocational nursing.
  • One year experience as a Licensed Vocational Nurse in a clinical care setting required.
  • Home health experience preferred.
