Hospice Nurse Specialist

We are seeking a compassionate and skilled Registered Nurse to join our Hospice team at ViaQuest. As a Hospice Nurse, you will play a vital role in providing high-quality patient care and support to individuals and their families during their end-of-life journey.
Key Responsibilities- Provide physical and emotional support to patients and their families, addressing their physical, emotional, and spiritual needs.
- Assume primary responsibility for a patient and family caseload, including assessments, planning, implementing, and evaluating phases of the nursing process.
- Administer and instruct the administration of medication, ensuring patient safety and comfort.
- Initiate communication with physicians, other hospice team members, and other agencies to coordinate care and ensure seamless transitions.
- Maintain current patient records and progress notes, documenting all interactions and interventions.
- Supervise LPNs and Hospice Aides, providing guidance and support to ensure high-quality care delivery.
- Minimum of an associate's degree or equivalent in Nursing from an accredited nursing program.
- RN license and current registration.
- Nursing experience, preferably within the Hospice specialty.
- Valid driver's license, car insurance, and reliable transportation.
- Willingness to travel throughout assigned service areas.
- Comprehensive training, including patient visits with a preceptor and Homecare Homebase and Relias.
