Job Summary

We are seeking a qualified Care Coordinator to join our team at one of our Intermediate Care Facilities (ICFs). The successful candidate will be responsible for ensuring that residents receive the highest level of care and support, in accordance with industry regulations and best practices.

Responsibilities:

  • ">
  • Coordinating active treatment programs and working closely with our interdisciplinary team to ensure seamless delivery of services.">
  • Developing personalized service plans (ISPs) that cater to the unique needs of each resident.">
  • Managing internal and external services, providing consultations to residents as needed.">
  • Supervising programming staff, fostering a collaborative and supportive environment.">
  • Overseeing CNA interactions, promoting positive connections between staff and clients throughout all programming activities.">

Requirements:

To be considered for this role, you must have a bachelor's degree in a related field, such as social work, psychology, or rehabilitation. Additionally, we require at least one year of experience working with individuals with intellectual and developmental disabilities, as well as supervisor and/or leadership experience. Strong communication and leadership skills are essential, along with a genuine love for teamwork and collaboration.
