Care Manager I

The Care Manager I provides case management services for CalAIM clients, ensuring the assessment and planning of services to meet their complex medical and psychosocial needs.
Key Responsibilities- Comprehensive Assessments: Conduct thorough assessments and ongoing re-assessments of clients, including psychosocial, physical, and mental health, environmental, and spiritual needs.
- Community Living Plan: Develop and initiate client-centered, comprehensive, and consistent plans with program guidelines and policies.
- Home Visits and Escort Services: Conduct home visits, acute hospital, and skilled nursing facility visits, as well as escort clients to medical and other appointments as clinically indicated.
- Community Services: Identify, arrange, and monitor appropriate community services based on a good knowledge of Medicare, Medi-Cal, and other entitlement programs.
- Care Management Relationship: Establish and maintain a care management relationship with clients and their significant others, offering respect, dignity, and support.
- Documentation: Document all case management activity regarding identified problems within 24-48 hours, adding any new problems to the care plan as needed.
- Required Skills: M.S.W. degree required, one year working with disabled adults and/or older adults, experience with medical and psychosocial problems of functionally impaired adults and older adults, and exceptional communication and presentation skills.
The Care Manager I position offers a competitive salary range of $64,261.50 annually, with opportunities for professional growth and development in a dynamic and innovative organization.
