Job Summary

We are seeking an experienced Case Management Supervisor to lead our Assertive Community Treatment (ACT) Navigator team at Horizon House in Wilmington, Delaware.

The ideal candidate will have a Master's degree with a major in psychology, counseling, social work, nursing, rehabilitation, or a related field and at least five years of experience in the Mental Health field. A minimum of three to four years of supervisory/management experience is also required.

Key Responsibilities:
  • Provide managerial oversight to the ACT/ICM Team and manage daily operations in compliance with state and local regulations and agency policies and procedures.
  • Develop, supervise, implement, and evaluate program systems for the ACT/ICM Team.
  • Lead and direct the team and staff, providing administrative supervision of ACT/ICM team personnel.
  • Schedule regular supervisory and staff meetings, case conferences, and staff training and development.
  • Coordinate, monitor, and ensure regulatory compliance for program service delivery.
  • Review and assess incoming participant referrals and assign to case managers; oversee caseload assignments and initiate redistribution as necessary.

Requirements:

  • Master's degree with a major in psychology, counseling, social work, nursing, rehabilitation, or a related field.
  • At least five years of experience in the Mental Health field.
  • Minimum of three to four years of supervisory/management experience.
  • Excellent communication skills.
  • Detailed-oriented and highly organized.
  • Ability to work independently and as part of a focused team.
  • Possess a valid driver's license with an acceptable driving record.
Work Environment:
  • This position requires sitting for long periods, frequent talking and listening, and may involve lifting, carrying, folding, kneeling, reaching, bending, stacking, and stooping.
  • Must be available for local travel and carry a cell phone for emergency situations.
