Job Summary

We are seeking an experienced Registered Nurse to join our team in Liberty, KY. As a Registered Nurse with LHC Group, you will have the opportunity to work in a dynamic and patient-centered environment.

About the Role

The Registered Nurse (RN) provides high-quality nursing care to patients in their homes, working closely with physicians and other healthcare professionals to develop and implement individualized care plans. This role requires strong clinical skills, excellent communication, and a passion for delivering exceptional patient care.

Duties and Responsibilities

- Conduct thorough initial and ongoing assessments of patients' physical and emotional needs, developing comprehensive care plans that address these needs
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care and ensure seamless transitions between settings
- Provide education and support to patients and families, empowering them to manage chronic conditions and maintain optimal health
- Utilize evidence-based practices and guidelines to deliver high-quality, patient-centered care

Requirements

- Current RN licensure in the state of practice
- Current CPR certification
- Reliable transportation and valid driver's license

Benefits

LHC Group offers a competitive salary range of $85,000 - $115,000 per year, depending on experience, as well as a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) retirement plan, and paid time off.
