Job Description: Our organization is seeking a skilled Care Coordinator to join our team. The successful candidate will be responsible for providing high-quality therapeutic interventions and services in support of an individual's wellness and recovery.

This role requires working with individuals who are chronically homeless, providing direct, face-to-face services and traveling throughout Solano County. You will assess individual risk status, intervene appropriately, and create service plans to help individuals achieve their goals.

The ideal candidate will have 4 years of experience in mental health/social services, a valid California driver's license, personal vehicle, and proof of insurance. Our organization offers a competitive salary of $27.00 per hour, increasing to $30.00 per hour after 90 days of satisfactory performance.

  • Develop and deliver high-quality therapeutic interventions and services in support of an individual's wellness and recovery
  • Work with individuals who are chronically homeless, providing direct, face-to-face services and traveling throughout Solano County
  • Assess individual risk status, intervene appropriately, and create service plans to help individuals achieve their goals
  • Utilize Evidence-Based Practice for the designated population/program
  • Continuously assess individual's risk status and intervene appropriately
