Job Overview

Compensation and Benefits

The estimated salary for this position ranges from $65,000 to $85,000 per year, depending on experience. In addition to competitive pay, we offer a comprehensive benefits package that includes nine paid holidays, generous PTO (four weeks your first year), medical insurance with a significant employer contribution towards the premium, employer-paid life/AD&D ($50k), short- and long-term disability, optional HSA, dental, vision, life/AD&D, and a 401(k) plan with employer contribution.

Job Description

The successful candidate will be responsible for providing patient education and linking pregnant women to care, support, and services that enhance prenatal care and promote their health and their baby's health.

Key Responsibilities
  • Recruit participants for initial intakes and referrals to appropriate home visitation programs such as Healthy Start, Nurse Family Partnership, Healthy Families, and Parents as Teachers.
  • Enroll participants not entered into Healthy Start home visitation services into Pregnancy Medical Home for Interconception Care Pathway.
  • Identify participant risk factors, concerns, priorities, strengths, and resources.
  • Conduct screenings to identify risk factors such as domestic violence, substance use/abuse, perinatal maternal distress, and/or mental health issues.
  • Enroll women with substance use disorder, mental health, or developmental delays into a Plan of Safe Care.
  • Make referrals to necessary resources as needed, such as WIC.
  • Provide education to participants using program materials to reinforce care plans implemented by the medical care team, promoting medical compliance and progress on medical goals.
  • Assist participants in accessing medical care and monitor the effectiveness of services, adjusting care plans as needed.
  • Ensure each participant schedules and completes postpartum visits.
  • Collect and enter data into designated databases and participate in quality improvement activities as directed.
  • Provide disease and chronic illness management education, referral, and follow-up services, including referrals to subspecialists and other needed services.
  • Offer instruction and education to help participants self-manage chronic, pregnancy-induced, or pregnancy-related medical conditions.
  • Provide information on medications, side effects, and risks of noncompliance.
  • Conduct necessary follow-up calls with participants to obtain required information, remind them of appointments, and provide additional education.
  • Complete all necessary programmatic and organizational forms, documents, and reports in a timely manner.
  • Participate in proactive team efforts to achieve departmental and company goals.
  • Provide leadership to others through example and sharing of knowledge/skills.
  • Adhere to all organization policies, practices, and procedures.
  • Perform other duties as assigned.
Requirements and Qualifications
  • A bachelor's degree in public health, social work, or a related field with 2-3 years of experience in maternal and child health, public health, behavioral health, pediatrics, or other fields, or a master's degree in public health, social work, behavioral health, or a related field with 1 year of experience in maternal and child health, public health, behavioral health, pediatrics, or other fields.
  • Proficient computer skills, including Microsoft Outlook, Word, Access, Excel, PowerPoint, and Internet applications.
