Job Summary:
The Care Coordination Specialist is responsible for facilitating the coordination of patient care across various departments and services. This role requires strong communication and organizational skills to ensure seamless transitions and optimal health outcomes for patients.

About the Role:
This position involves assessing patients' needs and developing individualized transition plans. The specialist will work closely with healthcare teams to identify opportunities for improving patient care and reducing readmissions. Key responsibilities include:
  • Conducting comprehensive psycho-social assessments and developing targeted interventions to promote timely throughput and safe discharge.
  • Collaborating with physicians, office staff, and ancillary departments to ensure accurate and effective communication.
  • Maintaining accurate and up-to-date patient demographic and insurance information.
  • Participating in department quality improvement initiatives to enhance patient care and outcomes.

Requirements:
To be successful in this role, you will need a Master's of Social Work degree and at least two years of acute hospital experience. Accredited Case Manager (ACM) certification is preferred. We also require active licensure or registration to practice as a social worker in the state of Massachusetts.

Salary and Benefits:
We offer an attractive salary range of $65,000 - $90,000 per year, depending on experience, plus comprehensive benefits packages including health insurance, retirement savings, and paid time off. If you are offered this position and must be vaccinated under any applicable law, you will be required to show proof of full vaccination or obtain an approval of a religious or medical exemption prior to your start date.
