Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Assessment team at Commonwealth Care Alliance. As a key member of our care team, you will conduct timely, high-quality, and accurate in-person assessments to identify members' concerns and unmet needs.

Key Responsibilities
  • Conduct in-person assessment visits and clinical exams to capture members' clinical needs and meet regulatory requirements.
  • Utilize electronic health records, scheduling platforms, and claims platforms to collect data, document member interactions, and communicate with the care team.
  • Escalate members with urgent needs to the Regional RN Supervisor and/or manager during or immediately following the visit.
  • Participate in assessing and improving quality within the scope of responsibilities and throughout the organization.
  • Collaborate with care team members to determine whether members need to be placed in a different program.
Requirements
  • Associate degree in Nursing
  • RN with an active registered nursing license in the Commonwealth of Massachusetts, in good standing.
  • 1+ years of clinical experience, including clinical rotation.
  • Experience with electronic medical records and care management platforms.
