About the Role:

Eliot Community Human Services is seeking a skilled Clinical Leader to oversee the clinical operations of the Health Home Outreach Care Team. This is an exciting opportunity to join a dynamic team and make a meaningful impact in the lives of our clients.

Key Responsibilities:

  • Provide supervision and clinical guidance on Annual MDS Comprehensive Assessment, Person-Centered Care Plan (PCCP), and the Member Engagement process.
  • Ensure coordination of services, assessments, and treatment planning to empower enrollees to exercise autonomy in all aspects of their life.
  • Provide overall clinical supervision and oversight of care coordination and service delivery of assigned Health Home Team(s).
  • Teach and model clinical interventions, strength-based strategies, and ensure services are culturally competent.

Requirements:

  • Clinical licensure required.
  • At least four years of experience working with people with severe and persistent mental illnesses, substance abuse disorders, or those with complex medical conditions.
  • At least two years supervisory experience.
