Palliative Care Registered Nurse Opportunity

We are seeking a compassionate and experienced Registered Nurse to join our team at Illumia Health, a leading provider in hospice and palliative care. Our clinical teams deliver advanced illness management, a specialized form of high-touch palliative care designed to provide relief from the symptoms and stress of illness while improving quality of life for patients in any setting.

Key Responsibilities:
Conduct in-person and telephonic assessments for palliative care patients
Complete risk stratification assessments and offer education to patients and families
Collaborate with the interdisciplinary team to coordinate care and support patient comfort
Communicate clinical findings and document timely clinical records

Requirements:
Registered Nurse license to practice in the state
2 years of prior skilled Registered Nurse experience
Nursing experience in palliative care, oncology, hospice, or geriatric care preferred
Strong compassionate phone skills and excellent written and verbal communication
