Job Summary

We are seeking a skilled Healthcare Transformation Specialist to join our Community Care team at CVS Health.

About the Role

This is an exciting opportunity for a professional with expertise in clinical operations and patient care management to make a meaningful impact on our members' health outcomes. As a Healthcare Transformation Specialist, you will work collaboratively with internal stakeholders and external partners to design and implement effective case management strategies that promote quality, cost-effective healthcare services.

Your key responsibilities will include:

  • Developing and implementing evidence-based clinical models to improve patient care and reduce healthcare costs
  • Facilitating integrative functions using clinical tools and information/data to optimize member wellness and medical outcomes
  • Collaborating with multidisciplinary teams to address complex healthcare needs and develop proactive solutions

You will be responsible for managing a caseload of high-risk members, conducting thorough assessments, and creating personalized plans to achieve optimal health outcomes. Your expertise in clinical coverage, network management, and policy development will enable you to drive transformative change within our organization.

Salary and Benefits

The typical salary range for this role is $60,522.80 - $129,615.20 per annum. In addition to your compensation, you can expect a comprehensive benefits package that includes medical, dental, and vision insurance, a 401(k) retirement savings plan, and access to our Employee Stock Purchase Plan. You will also enjoy a fully-paid term life insurance plan and short-term and long-term disability benefits.

Requirements

To be successful in this role, you will need:

  • An active TX Compact license as a Registered Nurse (RN)
  • A minimum of 3+ years of clinical experience, preferably in diabetes, CHF, CKD, post-acute care, hospice, palliative care, or cardiac care with Medicare members
  • A minimum of 2+ years of experience in case management, discharge planning, and/or home health care coordination
