Job Summary

Tennessee Quality Care – an Addus family company is seeking a skilled Physical Therapist to join our team in the Cookeville, TN area. As a key member of our Home Health Division, you will provide high-quality physical therapy services to patients and help them achieve their goals.

About the Role

This full-time position offers a competitive salary range of $85,000 - $115,000 per year, depending on experience. You will work closely with patients, families, and healthcare professionals to develop personalized treatment plans that address the unique needs of each individual. With a strong focus on patient-centered care, you will be empowered to make a meaningful difference in the lives of those you serve.

Responsibilities
  • Provide physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act.
  • Advise, consult, and teach appropriate treatment procedures and supportive activities to clients, family members, and other agency staff.
  • Assist the provider in evaluating level of function and participate in the development of the plan of care.
  • Prepare clinical and progress notes, and translate all exercises into functional activities or activities of daily living.
  • Develop needed plans for modifying equipment, appliances, and the physical surrounding in clients' homes.
  • Confer regularly with attending providers regarding client progress and report physical and emotional conditions and reactions to treatment.
  • Interpret to clients and families the implications of treatment consistent with the actions and wishes of the provider.
  • Provide families with information, support, and encouragement to help motivate them in their progress.
  • Supervise physical therapy assistants according to organization policy and state regulations.
Requirements

To qualify for this role, you must have:

  • A graduate degree from a School of Physical Therapy approved by the American Physical Therapy Association or Allied Health Education and Accreditation of the American Medical Association and the Physical Therapy Association.
  • Current licensure as a Physical Therapist in the state of practice.
  • At least two years' experience in an acute hospital or equivalent experience (preferred).
