About the Community Care RN Role

The Community Care Registered Nurse (RN) is a critical member of our healthcare team, providing and supervising professional nursing care for Providence patients. We strive to keep older adults as healthy as possible living in the community through clinics, home visits, and more.

We are seeking an experienced RN to join our team at Providence PACE, a Program of All-Inclusive Care for the Elderly. As a Community Care RN, you will apply your knowledge of assessing, planning, ordering, giving, delegating, teaching, and supervising professional nursing care delivery that promotes patients' optimal health and independence.

Key Responsibilities:

  • Provide high-quality nursing care to patients in their homes and communities.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Supervise and educate other caregivers on how to provide quality care.
  • Collaborate with healthcare providers and family members to ensure seamless care coordination.
  • Maintain accurate and up-to-date records of patient care and progress.

Required Qualifications:

  • Degree in Nursing.
  • Valid Oregon Registered Nurse License.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • Relevant certifications applicable to the position, as specified by the Nurse Manager.

Preferred Qualifications:

  • Bachelor's Degree in Nursing.
  • 1 year of Home health RN experience.
