RN Case Manager
Our RN Case Manager plays a critical role in collaborating with internal and external disciplines to direct and coordinate patient care from admission to transfer or discharge. This position is responsible for ensuring cost-effective utilization of hospital resources, minimizing out-of-pocket expenses for patients, and providing exceptional customer support to internal and external customers.
Responsibilities- Identify discharge needs and determine the appropriate level of care for patients, coordinating planning and problem-solving to ensure seamless continuum of patient care and prevent readmission to the hospital.
- Ensure all relevant clinical information is provided to outside agencies and communicate pertinent information to the patient care team effectively and professionally.
- Take ownership of identifying and eliminating barriers to effective patient stay management and promote improvements in work processes within the department, hospital, and community.
- Responsible for professional development and maintaining licensure, taking ownership of self-education and advocating for patient legal, social, and ethical rights while balancing patient resources and hospital financial stewardship.
- Refer quality issues and poor utilization of services to the appropriate sources and coordinate/facilitate patient/family education.
- Bachelor's degree from an accredited RN program.
- Current license to practice as a registered nurse in the State of Ohio.
- Minimum of 3-5 years recent clinical experience in an acute care setting.
