Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our Home Health Care team. As a LPN, you will provide high-quality patient care and perform technical skilled care in the patient's home.
Key Responsibilities- Provide quality patient care and perform technical skilled care in the patient's home
- Communicate effectively with physicians, patients, family members, and agency staff regarding patient care issues or needs
- Document patient visits thoroughly and completely per policy and payer requirements
- Actively participate in the performance improvement plan and work to reduce patient hospitalizations and improve outcomes
- Current LPN licensure in state of practice
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
- CPR Certification required
