Nurse Advocate

We are seeking a compassionate and skilled Registered Nurse to join our team as an After-Hours Hospice RN. As a key member of our care team, you will provide high-quality patient care and support to patients and their families during after-hours periods.
Responsibilities- Provide patient-centered care and advocate for patients' needs and preferences
- Collaborate with the hospice care team to develop and implement individualized care plans
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions
- Participate in on-call rotations to provide 24/7 coverage and support
- Graduate of an accredited school of nursing with a current state license as a registered nurse
- 2+ years of nursing experience in a clinical care setting, with hospice experience preferred
- Ability to sit, stand, bend, move intermittently, and lift at least 25 lbs
