Job Description

We are seeking a compassionate Physical Therapist to join our team. As a Physical Therapist at JAG Physical Therapy, you will be responsible for assessing patients' strengths and weaknesses, creating effective treatment plans, and maintaining patient confidentiality.

You will work in a positive, high-energy environment, employing your skills to facilitate healing, recovery, and functional independence for patients. This position requires a licensed physical therapist to work collaboratively with an interdisciplinary team, providing patient care and documentation to ensure quality and compliance.


Required Skills and Qualifications
  • Bachelor's or Master's degree required; Doctorate degree preferred
  • Licensed Physical Therapist in the state practicing in
  • Completed state-regulated CEU requirements in the appropriate state (NY, NJ)
  • Valid CPR license
  • Experience/knowledge with EMR software preferred
  • Excellent oral and written communication skills
  • Strong clinical decision-making skills
  • Excellent work ethic and dedication to patient success
  • Basic computer skills
  • Ability to work as a team member and individually with minimal supervision
  • Maintains a positive, professional demeanor at all times

Responsibilities
  1. Collections and records patient care data that follows format and guidelines of the patient care service, regulatory agencies, and third-party payers, reflecting an understanding of reimbursement systems and their impact on patient care.
  2. This includes initial evaluation, daily notes, reassessments/progress reports, discharge plans, and communication with members of the team.
  3. Records daily billing and follows department guidelines for productivity.
  4. Prepares for evaluations in accordance with department policies/procedures and standards of practice, including obtaining patient history, exhibiting knowledge of patient diagnoses, and researching unfamiliar diagnoses.
  5. Selects and administers tests and measures.
  6. Synthesizes evaluation results to formulate the diagnosis, goals, prognosis, and plan of care.
  7. Establishes treatment plans based on evaluation results, goals, and best practices.
  8. Collaborates with interdisciplinary teams, patients, and patient support systems.
  9. Assesses and re-assesses the need for modifications to the plan of care and goals.
  10. Identifies the need for consultations and/or refers patients to another healthcare professional or healthcare services.
  11. Initiates and completes discharge plans.
  12. Provides education/training to patients and/or patient support systems.
  13. Functional training includes but is not limited to ADLs/IADLs, therapeutic activities (e.g., transfers, bed mobility), rehabilitation equipment prescription, and training, and community reintegration.
  14. Responsible for treatment, plan of care, exercise prescription for patient caseload.
  15. Provides interventions to patients that are safe, effective, and in compliance with JAG.
  16. Musculoskeletal interventions include but are not limited to therapeutic exercise, joint mobilization, and modalities.
  17. Provides data, note writing using WEB PT EMR for all treatments provided.
  18. Oversees support staff of PTA, ATC, PT aides.
  19. Attends annual JAG Billing and Coding seminar.
  20. Promotes JAG in the community through lectures/presentations.
