Diabetes Education Specialist - Healthcare Professional
About the Role:
We are seeking a highly skilled Diabetes Education Specialist to join our team at Tryon Medical Partners. As a key member of our Value Based Medicine team, you will play a vital role in providing expert guidance to patients with diabetes on improving their health. Your extensive knowledge and experience will enable you to inform patients about effective diabetes management strategies, including diet, medication, and physical activity.
In this full-time position, you will work closely with patients to develop personalized plans for maintaining a healthy lifestyle and effectively managing the disease. You will also be responsible for teaching patients about diabetes management and prevention, developing customized plans for healthy eating and blood glucose monitoring, and training patients on the use of equipment such as insulin pumps, continuous glucose monitors, and blood glucose meters. Additionally, you will provide patients with valuable information on healthy food choices and meal planning, monitor their progress, and adjust treatment plans as needed to ensure optimal outcomes.

Key Responsibilities:
- Teach patients about diabetes management and prevention.
- Develop personalized plans for patients to maintain a healthy diet and monitor blood glucose levels.
- Collaborate with patients to increase physical activity and improve overall health.
- Train patients on the use of equipment such as insulin pumps, continuous glucose monitors, and blood glucose meters.
- Provide patients with information on healthy food choices and meal planning.
- Monitor patients' progress and adjust treatment plans as needed.
Requirements:
Education and Certifications:
- A high school diploma or equivalent.
- A degree in nursing, nutrition, pharmacy, occupational therapy, or social work.
- Certification as a diabetes educator.
- Completion of Hepatitis B Vaccine and Flu Vaccine Form.
- Completion of TB test.
Experience:
- At least 2 years of experience working with patients with diabetes.
- 1,000 hours of diabetes self-management education required for certification as a diabetes educator.
- A minimum of 15 hours of continuing education coursework annually related to diabetes treatment and care.
Estimated Salary Range: $60,000 - $80,000 per year
