Job Responsibilities

The Clinical Care Collaboration Specialist works with an Advanced Practice Nurse (APN) who conducts home and/or telehealth wellness assessments on patients designated by HarmonyCares Medical Group. The APN will not have prescriptive authority or treat patients. As a Clinical Care Collaboration Specialist, you will:

  • Be available via telecommunication or in-person for consultation with the APN to address patient assessment questions.
  • Regularly review patient charts annually.
  • Consult with the APN to ensure adherence to policies, procedures, state laws, and quality standards.
  • Provide exceptional customer service to all external and internal customers.

 

Qualifications

To be successful in this role, you must have:

  • An active and unrestricted medical physician license.
  • Board certification or eligibility required.
  • Completion of a medical residency program.
  • A minimum of 1 year of experience in healthcare management.
  • Experience with risk adjustment.
  • Residency in the state where the opening exists.

 

Pay Transparency

The compensation package for this position will be based on factors unique to each candidate, including their skill set, experience, qualifications, and job-related considerations. Estimated salary: $120,000 - $180,000 per year, depending on location.
