We are seeking a skilled Healthcare Transition Specialist to join our team at Suncoast Center, Inc.

The ideal candidate will possess excellent communication and organizational skills, with the ability to work effectively in a fast-paced environment.

About the Role

This role will involve serving as the main point of contact for transitions of care between care coordinator partners, providing direct linkage to outpatient care for high-risk and recently discharged behavior health clients, and participating in treatment planning, safety planning, caring contacts, and follow-up services as needed.

Responsibilities
  • Serve as the primary liaison for transitions of care between care coordinator partners.
  • Provide direct access to outpatient care for high-risk and recently discharged behavior health clients.
  • Participate in treatment planning, safety planning, and caring contacts to ensure seamless transitions of care.
  • Update client electronic health records as necessary.
  • Stay up-to-date on training, resources, and best practices for safe transitions of care.
  • Document all interactions according to agency standards.
Requirements
  • Bachelor's degree in Public Health, Psychology, Social Work, or a related field of study.
  • Experience with care coordination, case management, and/or patient discharge processes.
  • Familiarity with electronic health records (preferable).
  • Knowledge of safety planning practices and other elements of the Zero Suicide framework (preferable).
  • Must be at least 21 years of age.
  • Negative drug screening prior to hire and throughout employment.
  • Clear criminal background check throughout employment with Suncoast Center and local checks prior to hire and annually along with DCF Level II prior to hire and as required by DCF, and must have a valid Florida driver's license with an acceptable driving record maintained throughout employment along with reliable vehicle, valid registration, and valid vehicle insurance.
Estimated Salary: $45,000 - $65,000 per year
