Responsibilities:
- Develop and implement personalized care plans for clients, in collaboration with their healthcare team.
- Conduct home visits to assess clients' needs and provide high-quality care.
- Administer medications, perform wound care, and provide other treatments as needed.
- Monitor clients' conditions and report any changes to the Clinical or Client Services Manager.
- Document all interactions and interventions in a timely and accurate manner.
Qualifications:
- A current license as a Registered Nurse in Pennsylvania.
- A minimum of one year of recent, verifiable clinical experience.
- Prior home care experience is strongly preferred, but not required.
- Graduation from an accredited and approved nursing program.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills and the ability to communicate effectively with clients and their families.
