Job Summary

PAM Health Rehabilitation Hospital of Georgetown seeks a skilled Clinical Navigator to cultivate referral relationships, manage assigned territory, and complete patient assessments. This role works under the direction of the Director of Business Development, collaborating with the business development team, admissions staff, and internal and external stakeholders to facilitate referral conversion.

Responsibilities

  1. Pre-Admission Assessment: Maintain responsibility for patient referral until admission to Post Acute Medical, displaying knowledge of inpatient rehabilitation and long-term acute care admissions criteria.
  2. Facilitate referral conversions using the fast track process, completing phone/FAX assessments as needed.
  3. Document pre-admission assessments accurately and completely, reviewing medical records for pre-certification financial coverage and admission criteria.
  4. Serve as a navigator to the Admissions office, providing clinical information for pre-certification and communicating admission status to referral sources.
  5. Educate and support family members of current, past, and potential patients, communicating with facility Admissions and CEO to optimize census management and case finding strategies.

Business Development

Design and manage marketing strategies for assigned territory, using sales and marketing techniques to show growth in admission and revenue volume. Create a Quarterly Business Development Plan, identifying contacts and managing potential referral sources using account management systems. Utilize educational opportunities to present the hospital to referral sources, meeting referral and admission goals for assigned territories.

Documentation and Reporting

Effectively maintain a log of all pending cases, providing periodic updates on activities and market trends. Maintain a resource library of articles and information for distribution to professionals, community members, families, and potential patients.
