Job Summary:

Lifespark Holdings, Inc. is seeking a highly skilled and compassionate Clinical Liaison LPN to join our team. As a key member of our senior care team, you will play a vital role in providing exceptional patient care and support to our clients.

Key Responsibilities:

  • Serve as the primary point of contact for field-based healthcare professionals and clients, ensuring seamless communication and coordination of care.
  • Support clinical staff in delivering high-quality, whole-person senior primary care services, leveraging a top-of-license model to maximize vocational satisfaction and efficiency.
  • Collaborate with healthcare professionals to develop and implement personalized care plans, ensuring the best possible outcomes for our clients.
  • Utilize technology, including electronic medical records and clinical software platforms, to streamline communication and care coordination.
  • Provide exceptional customer service, responding to client needs and concerns in a timely and professional manner.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.

Requirements:

  • Licensed Practical Nurse diploma or certified medical assistant.
  • Experience using technology, including Microsoft Office, smartphones, calendaring, and clinical software platforms.
  • Previous hospital or clinic experience highly recommended.
  • Strong planning, creativity, and problem-solving skills.
  • Excellent communication and interpersonal skills, with the ability to effectively communicate at all levels of the organization.
  • Ability to maintain composure and stay positive in stressful situations.
  • Strong organizational and prioritization skills, with the ability to manage multiple demands in a fast-paced environment.
  • Collaborative spirit and desire to be part of a team.
  • Willingness to travel to multiple senior living sites across the Twin Cities metropolitan region.
