About the Role

Charles River Community Health is seeking a highly motivated and compassionate Integrated Community Health Worker to join our team. As a frontline public health worker, you will serve as a liaison between health and social services and the community, facilitating access to services and improving the quality and cultural competence of service delivery.

Key Responsibilities
  • Establish and maintain trusting relationships with patients and their families, providing general support and encouragement.
  • Provide ongoing follow-up, basic motivational interviewing, and goal setting with patients/families.
  • Serve as a liaison between clinical staff and patients, following up on appointments, referrals, and needed resources.
  • Help patients set personal goals and attend appointments.
  • Provide referrals for services to community agencies as appropriate.
  • Connect patients with transportation resources and needed resources to carry out their plan of care.
  • Work closely with the integrated medical team to ensure comprehensive and coordinated care.
  • Remain knowledgeable about community resources and continuously validate and evaluate their effectiveness.
  • Provide consistent communication to medical providers and nurses to evaluate patient/family status and report progress and compliance with identified plans of care.
  • Enable and improve culturally and linguistically competent services and care by understanding community culture and resources.
  • Conduct outreach and implement programs in the community to promote, maintain, and improve community health needs and provide resources.
  • Collect data to identify community health needs.
  • Advise community groups on chronic disease and diabetes self-management, hypertension, and nutrition.
  • Advise clients on general health and exercise and self-care activities.
  • Identify age-appropriate needs and resources.
  • Follow up on hospitalizations and no-shows.
  • Provide situation-appropriate social support to patients using experience lived and learned through training provided at CRCH.
  • Provide informal counseling as needed.
Requirements
  • Bachelor's Degree in a related field required.
  • Bilingual speaking preferred in Portuguese, Spanish, or Haitian Creole, but not required.
  • BLS or CPR Certification required, must be completed prior to start date, and must be hybrid or practical certificate including adult, pediatric, and AED trainings.
  • Community Health Worker Mass Board certification preferred, not required. Must obtain within 3-4 years of hire date.
  • Preferred Medical Interpreter Certification.
  • Must have knowledge of computer systems such as Word and Excel.
  • Strong interpersonal, organizational, and time management skills required.
  • Must have a willingness to work flexible hours to meet the organization's needs/demands.
  • Must have excellent communication skills, particularly with people from diverse cultures, with the ability to understand the community, population, and patients we serve.
  • Must have experience in diverse cultures, with a strong commitment to promoting Diversity, Equity, and Inclusion and reducing inequities.
  • Must believe in the work we do at CRCH, with a strong passion to serve underserved populations in diverse settings.
  • Knowledge of both community and healthcare system resources.
  • Experience serving the population(s) that will be referred for patient navigation services.
  • Excellent written and oral communication skills.
  • Experience in diverse settings preferred.
