About the Role
We are looking for a dedicated and compassionate individual to work closely with patients, nurses, and other healthcare professionals to deliver exceptional care and improve patient outcomes. Your primary responsibilities will include conducting patient assessments, developing personalized care plans, and collaborating with multidisciplinary teams to ensure seamless patient transitions.
Key Responsibilities
- Conduct thorough patient assessments to identify care needs and develop effective care plans
- Collaborate with patients, families, and healthcare teams to provide comprehensive care and support
- Administer medications and treatments as prescribed by physicians, ensuring accurate dosing and documentation
- Monitor patient vital signs, track treatment progress, and report any concerns or issues to physicians and nursing staff
- Maintain accurate and detailed patient records, adhering to regulatory requirements and company policies
- Participate in continuous quality improvement initiatives, contributing to process improvements and best practices
- Stay up-to-date with industry developments, regulations, and evidence-based practices, applying this knowledge to enhance patient care and services
Requirements and Qualifications
- Bachelor's degree in Nursing or related field (RN license required)
- Minimum 1 year of experience in renal care, acute care, or related field (gerontology, pediatrics, etc.)
- Excellent communication and interpersonal skills, with ability to work effectively with diverse patient populations and healthcare teams
- Strong assessment and critical thinking skills, with ability to analyze complex patient information and make informed decisions
- Ability to work independently, prioritize tasks, and manage multiple responsibilities in a fast-paced environment
- Proficiency in electronic medical records (EMRs) and other software applications
- High school diploma or equivalent; if RN license not held, certification in phlebotomy, EMT, or related field may be considered
