We are seeking a dedicated Physical Therapist to join our team at an esteemed healthcare facility in Salinas, California. This is an exceptional opportunity to enhance your physical therapy skills while advancing your career as a skilled nursing professional.
Job Summary: We require a talented Physical Therapist with experience in Skilled Nursing Facilities to work in our dynamic environment. Your role will involve providing comprehensive therapy and rehabilitation services to patients at the facility. As a skilled professional, you will work closely with new and ongoing patients to create personalized plans of treatment that include targeted exercises, procedures, and techniques to promote effective recovery.
Key Responsibilities:
- Evaluate patients using clinical reasoning to identify diagnoses and develop individualized plans of care.
- Perform age-appropriate competency skills tailored to each patient's unique needs.
- Follow established intervention protocols and adjust treatment plans as necessary.
- Document patient progress according to approved medical and facility documentation procedures.
- Discharge patients when goals have been met or medical necessity no longer warrants continued intervention.
- Collaborate with supervisors and peers to contribute to the overall productivity of the team.
Requirements:
- Active state license as a physical therapist
- Current BLS/CPR certification
- 6 months of PT experience within the last 5 years
