Job Description

At Phaxis, we are seeking a highly skilled Clinical Nurse Specialist to join our team. This is a unique opportunity to work in a dynamic environment and make a real difference in the lives of our clients.

About the Role

We are looking for a dedicated and compassionate nurse who can provide high-quality care to individuals in residential settings. As a Clinical Nurse Specialist, you will be responsible for providing case management and clinical services to our clients, including nursing notes, consultations, and medication management.

Responsibilities
  • Provide case management and clinical services to clients in residential settings
  • Develop and implement individualized care plans to meet the unique needs of each client
  • Conduct regular assessments and evaluations to ensure optimal health outcomes
  • Collaborate with interdisciplinary teams to coordinate care and services
  • Document all interactions and interventions in client records
Requirements
  • Degree in Nursing (BSN or higher)
  • Current LPN licensure
  • Minimum 2 years of experience in home health or similar setting
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
Working Conditions

This position requires frequent travel to residential settings throughout the city. You must have a reliable vehicle and a valid driver's license to be considered for this role.

Salary and Benefits

The estimated salary for this position is $65,000 - $80,000 per year, depending on experience. We also offer a comprehensive benefits package, including health insurance, retirement plan, and paid time off.
