
Medical Withdrawal Stabilization Coordinator

We are seeking a highly skilled Medical Withdrawal Stabilization Coordinator to join our team at BayMark Health Services.
About the RoleAs a Medical Withdrawal Stabilization Coordinator, you will play a vital role in ensuring the smooth intake process for patients undergoing medically managed withdrawal stabilization. Your primary responsibility will be to screen and assess patients to determine their eligibility for our services, develop tailored discharge plans, and facilitate effective communication with hospital staff.
This is a challenging yet rewarding position that requires strong interpersonal skills, attention to detail, and the ability to work effectively in a fast-paced environment.
Key Responsibilities- Screen patients via phone and walk-in inquiries, initiating insurance verification, scheduling assessments, and intakes
- Complete all paperwork clearly, concisely, and in line with policy and procedure
- Maintain effective communication with hospital staff (physicians, nursing, etc.) regarding patient assessment, continued stay, and discharge planning
- Ensure HIPAA compliance
- Perform facility rounds and follow-up calls on patients daily according to policy and procedure
- Develop and implement discharge and aftercare plans in collaboration with patients
- Report daily census to the corporate office
- Conduct community outreach telephone calls daily to various agencies
- Complete bio-psychosocial assessments
To succeed in this role, you should have:
- A Bachelor's degree in Human Services or a related field, with at least 2 years of experience in social service and/or clinical work
- In-depth knowledge of addiction and/or dependence
- Previous experience as a substance abuse counselor or case manager is preferred, particularly in settings involving MAT treatment or detox
- Excellent teamwork and communication skills, with the ability to work independently and adapt to changing priorities
- The capacity to identify problems and implement solutions in a fast-paced environment
BayMark Health Services offers a competitive salary range of $50,000 - $70,000 per year, depending on experience, along with a comprehensive benefits package, including medical, dental, vision, and 401(k) retirement savings plan.
