Job Summary:
We're seeking a skilled In-Home Healthcare Specialist to join our team. As an In-Home Healthcare Specialist, you will conduct in-home assessments of patients, perform evidence-based practice screenings, and identify diagnoses to inform care management.
Key Responsibilities:
- Conduct comprehensive in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
- Perform evidence-based practice screenings, including point-of-care testing as appropriate.
- Identify diagnoses to be used in care management and active medical management in furtherance of treatment.
- Communicate findings from assessments to inform member's primary care physician of potential gaps in care.
- Educate members on topics such as disease process, medication, and compliance.
Requirements:
- Active NP license or ability to obtain by start date; licensure must be unencumbered and free of any open/unresolved disciplinary actions.
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain national certification and/or NP license in state of assignment by start date.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag in and out of the car, navigating stairs and various dwelling conditions, and sitting, standing, and kneeling to perform physical assessment.
- Reliable transportation for completing home visit assessments.
