We are seeking compassionate and dedicated Home Health Nurses to join our team at Aveanna Healthcare. As a leader in home health care services, we provide personalized support to medically fragile children and adults, empowering them to live their best lives.

About the Role:

This is an excellent opportunity for nurses who want to make a positive impact on patients' lives while enjoying flexibility and work-life balance. As a Home Health Nurse with Aveanna, you will be responsible for providing one-on-one care to patients in the comfort of their own homes. Your expertise will help patients manage chronic conditions, recover from illnesses, or simply maintain their independence.

Key Responsibilities:

* Develop and implement patient-centered care plans in collaboration with healthcare teams
* Provide skilled nursing care, including wound care, medication management, and vital sign monitoring
* Educate patients and families on disease management, self-care techniques, and medication adherence
* Collaborate with multidisciplinary teams to ensure seamless care transitions

Requirements:

* Active, unencumbered RN/LPN/LVN license in the state where you will practice
* Current CPR certification from American Red Cross or American Heart Association
* TB skin test (current within the last 12 months)
* At least six months of prior hands-on nursing experience (preferably in home health care)
* Reliable transportation for frequent travel to patient locations
