At ACCELerate Willow Grove - Hatboro, PA, we are seeking a dedicated Social Services Specialist to join our team. As a Social Services Specialist, you will work collaboratively with patients, residents, and their families to identify strengths, social, emotional, and mental health needs. Your goal will be to provide patients with the highest practical level of physical, mental, and psychosocial well-being and quality of life.

Key Responsibilities:
  • Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
  • Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
  • Hold meetings with patients/residents, families, and significant others as requested to provide support and information to improve quality of life.
  • Perform patient/resident discharge planning activities, including family/patient education, nursing center documentation, and referrals to appropriate agencies and suppliers.
  • Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.

Qualifications:
  • Bachelor's degree in social work or human services field required
  • Must possess any certifications/licensures as required by State of employment to practice in long-term care
