Job Summary

This position is responsible for performing functions related to the screening, assessment, and benefit/service coordination for Vulnerable Adults who have been identified as potentially needing protective services.

Key Responsibilities
  • Engage in follow-up on reports of alleged abuse, neglect, or financial exploitation of a Vulnerable Adult and enter this information into a computer database.
  • Interview reporters and collaterals to gather critical information to assess each report and determine the Vulnerable Adult status of the alleged victim.
  • Mobilize community resources to provide protective services to remedy the reported maltreatment and reduce the risk of further abuse, neglect, or financial exploitation.
  • Coordinate data sharing with Law Enforcement and Lead Agencies.
  • Assess and monitor quality assurance of formal and informal services as required in accordance with applicable contacts, service plans, and assessed needs to ensure health and safety.
  • Engage and collaborate with medical/behavioral health providers, law enforcement, court, and other interested parties.
  • Assist Vulnerable Adults and interested parties in identifying and applying for needed benefits and services that will potentially remediate concerns or risk factors contributing to maltreatment.
  • Complete federal, state, and agency required documentation for all persons assessed, and upon transfer/closure of services.
  • Attend regular and periodic meetings and trainings consistent with the role and participate on work teams as requested.
  • Other duties as assigned such as supporting community emergency response functions per county emergency response plan.
Requirements
  • Bachelor's degree in one of the following fields: Social Work, Psychology, Sociology, Human Services, Special Education, or a related Human Services field.
  • One year full-time professional experience OR two years full-time para-professional experience in a human service agency.
  • Valid Driver's license.
Preferred Qualifications
  • Master's degree and/or experience in Social Work, Psychology, Sociology, Special Education, or a related field.
  • Prior professional experience in home or community-based settings.
  • Knowledge of or experience coordinating or monitoring Home and Community Based Service Waivers.
  • Knowledge of or experience coordinating or monitoring MN Public Assistance Benefits.
  • Knowledge of or experience working with Adult Protective Services to ensure the health and safety of Vulnerable Adults.
Work Environment

Work is performed in a Dakota County office building and on visits to client's homes. Travel is required. Lifting requirement of up to 20 pounds on an occasional basis. Exposure to hazardous physical conditions such as environmental conditions of unsanitary homes and property, unsafe neighborhoods, challenging and potentially violent and unpredictable clients and family members, pets, and animals.
