Job Summary

We are seeking a Patient Care Specialist to join our Behavioral Health team in Peabody, Massachusetts.

About the Role

This is an ongoing nursing position that requires 24 hours of work per week, with shifts spanning 8 hours and taking place in the evenings. As a Patient Care Specialist, you will be responsible for managing all aspects of patient care and providing family-centered care to patients serviced in your assigned department.

To be successful in this role, you must have 2+ years of experience in behavioral health and hold a valid/current license to practice as a RN in the Commonwealth of Massachusetts. A Bachelor's Degree is preferred, and BLS certification is required.

  • An estimated salary of $85,000 - $110,000 per year, based on experience and qualifications.

Responsibilities
  1. Manage all aspects of patient care, including assessing patient needs, developing and implementing individualized care plans, and evaluating patient progress.
  2. Provide education and support to patients and their families, promoting health literacy and empowering them to take an active role in their care.
  3. Collaborate with multidisciplinary teams to ensure seamless communication and coordination of care.
  4. Participate in quality improvement initiatives to enhance patient outcomes and experiences.
