About the Role

We are seeking a compassionate and experienced Nurse Practitioner to join our team at Abode Care Partners, where you will provide personalized medical care to seniors and individuals with complex needs in various settings.

Job Responsibilities
  • Develop and implement evidence-based medical care plans, taking into account patients' unique needs and circumstances.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care and maintain accurate medical records.
  • Perform routine check-ups, examinations, and treatments as needed, utilizing your expertise in chronic and urgent episodic illnesses, minor injuries, and other health issues.
  • Order and interpret laboratory and diagnostic testing, as well as refer patients to specialists when necessary.
Qualifications
  • Active state license as a Nurse Practitioner (APRN)
  • Current National Board Certification
  • Active DEA licensure
  • Master's degree in Nursing (MSN)
  • A minimum of five years of nursing experience, preferably two years as a Nurse Practitioner
  • Excellent communication and interpersonal skills, both verbally and written
  • Strong time management and organizational skills, with accountability for daily workflow
  • Proficiency in medical instruments and equipment
  • Knowledge of HIPAA and OSHA regulations
  • Demonstrated proficiency in computer-based data management programs, electronic medical record systems, and Microsoft Office applications
  • Licensed driver with valid automobile insurance

Salary Range: $60,000 - $72,000 per year
