Care Coordinator Speech Pathologist

We are seeking a dedicated Care Coordinator to work as a Speech Pathologist with Providence Sound HomeCare & Hospice. As a member of our team, you will be responsible for providing high-quality patient care and support to clients in need.

Responsibilities:
You will provide speech pathology services to clients in their homes, focusing on improving communication and swallowing disorders. Your duties will include assessing patients' needs, developing individualized treatment plans, and delivering evidence-based interventions. You will also communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
Requirements:
- A bachelor's degree in speech-language pathology or a related field.
- Current licensure as a speech-language pathologist (SLP) in the state where you will be working.
- At least 2 years of experience in speech pathology, preferably in home health or hospice settings.
- Excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
