Clinical Care Specialist

The Clinical Care Specialist will provide comprehensive management of patients within their given specialty and according to privileges approved by the Medical Staff office. The specialist has responsibility, authority, and accountability for the provision of care through collaboration with a physician. The specialist manages and provides patient care activities for a group of patients and their families through the application of independent judgment, communication, and collaboration with all team members. The specialist establishes and maintains collaborative relationships with physicians, other healthcare providers, patients, and their families to achieve desired patient outcomes throughout the continuum of care.
Responsibilities
• Collaboration: Interacts with various members of the healthcare team to meet patient care needs and collaborates with other providers to identify changing patient needs.
• Identifies teaching/learning needs of patients, documents patient care services in the electronic health record.
• Communication: Exhibits skill in the management of conflict situations, functions so as to promote trusting relationships, and demonstrates open and honest communication at the unit level.
• Participates in performance improvement activities as appropriate and promotes and fosters an atmosphere of teamwork and excellence among staff.
• Job Skills: Conducts physical examinations, initiates testing and treatment based on scope of practice, identifies normal and abnormal findings, and acts upon abnormal findings accordingly.
• Refers patients to appropriate specialists and/or allied health professionals and provides counseling and instruction regarding common patient problems.
• Safety: Protects patients, visitors, and employees by adhering to infection-control policies and protocols, and identifies patients using two identifiers (i.e., name and date of birth) prior to providing any treatment or procedure.
Knowledge, Skills & Abilities
• Heavy Work: Exerting up to 100 lbs. of force occasionally and/or up to 50 lbs. of force frequently and/or in excess of 20 lbs. of force constantly.
• Previous experience in a clinical setting is preferred.

• CRP Certification within Probationary Period (Required)
