Job Description

At Association House of Chicago, we are seeking a skilled Transition Support Specialist to join our team. As a key member of our community housing team, you will play a vital role in assisting individuals residing in nursing facilities or SMHRF care to prepare for and implement steps to transition to permanent supportive housing in the community.

Key Responsibilities:

  • Provide case management and therapeutic interventions to facilitate illness self-management, skill building, identification of natural supports, and use of community resources.
  • Use intensive outreach strategies to engage participants in services and monitor their symptoms and medications regularly.
  • Implement crisis services in consultation with supervisor/psychiatrist and ensure participants' safety and timely access to services.
  • Serve as a liaison with other systems of care, including nursing facility staff and collaborating treatment providers.
  • Document all services using program standards.

Requirements:

  • Bachelor's degree from an accredited college or university in psychology, social work, or a related field (or) High School diploma or GED plus 5 years supervised experience in the human service field.
  • Experience working with individuals with mental health, substance use treatment, or I/DD services preferred.
  • Experience working with diverse ethnic populations required.
  • Proficient in the use of computer software, including basic computer and data entry skills.
  • Personal vehicle, Illinois current driver license, and insurance, with a good driving record acceptable to our agency's insurance carrier.
  • Flexibility to work on weekends, evenings, and holidays during emergencies.
