We are seeking a skilled Resident Service Coordinator to join our team at Westwood Terrace in Moline, IL. As a Resident Service Coordinator, you will be responsible for providing exceptional customer service, social services, and support to our residents.



Job Summary

The successful candidate will have a bachelor's degree in a social service-related field and a minimum of two years of experience in social service delivery. You will possess excellent communication skills, be able to work independently, and demonstrate autonomy.



Key Responsibilities


  • Conduct comprehensive non-clinical assessments of residents to identify their needs and provide referrals to relevant services.
  • Develop and maintain partnerships with local community-based organizations, Area Agencies on Aging, and other stakeholders to ensure seamless service delivery.
  • Provide education and preventative health programs for residents, promoting healthy living and independence.
  • Monitor and follow up on services provided to residents, ensuring they receive the necessary support and resources.
  • Maintain accurate records of resident interactions, services provided, and outcomes.


Requirements

To be successful in this role, you will need:




  • A bachelor's degree in a social service-related field
  • Minimum two years of experience in social service delivery
  • Excellent communication and interpersonal skills
  • Ability to work independently and demonstrate autonomy
  • Bilingual skills are an asset


Benefits
