Job Summary

As an Independent Optometrist, you will have the autonomy to manage your own practice and patient care services within a Sam's Club facility. This role requires a Doctor of Optometry (OD) degree from an accredited optometry school and a valid state optometry license.

Key Responsibilities
  • Provide eye and vision care services to patients
  • Establish your own days of practice and hours of operation
  • Control your patient schedule and set your fee schedule
  • Keep 100% of exam/professional fees
  • Optional equipment/instrumentation and administrative support services
Requirements
  • Doctor of Optometry (OD) degree from an accredited optometry school
  • Valid state optometry license
  • General Liability Certificate
  • Professional Liability Insurance Certificate
  • W-9
