Job Description

The RN Special Care Unit Care Coordinator is a key member of our interdisciplinary team, responsible for overseeing and coordinating patient care on the special care unit. This role requires strong leadership skills, excellent communication abilities, and a passion for delivering high-quality patient care.

Key Responsibilities:

  • Supervise and direct daily functions of unit nurses and CNAs to ensure effective patient care
  • Chart accurately and in a timely manner, reflecting patient progress and needs
  • Provide and coordinate patient care services, promoting optimal physical, mental, and psychosocial well-being
  • Administer medication as ordered by physicians, ensuring accuracy and safety
  • Respond to emergency situations, prioritizing patient and staff safety
  • Develop and implement patient care plans, collaborating with interdisciplinary teams
  • Maintain professional relationships with patients, families, and colleagues, fostering a positive work environment
Required Skills and Qualifications

To be successful in this role, you will need:

  • A nursing diploma (associate's or bachelor's degree in nursing) and an active Registered Nurse (RN) license in good standing
  • At least one year of geriatric nursing experience, preferably in a post-acute care setting
  • Expert knowledge in field of practice, with ability to make independent decisions when necessary
  • Strong communication and interpersonal skills, with ability to work effectively in an interdisciplinary team
