Based in New York City, NY, we are seeking a skilled Nurse Home Visitor to join our team. As a member of the Bureau of Maternal Infant and Reproductive Health, you will play a vital role in supporting the health and well-being of women and their families.

Job Description

This is an exciting opportunity to work on transformative public health programming and infrastructure. As a Nurse Home Visitor, you will apply the nursing process to assess the physical, emotional, social, and environmental needs of women and their families. You will provide counseling and support to help them establish realistic goals and measurable outcomes, and develop a plan of care that promotes trust and problem-solving.

You will also link women and families with community resources relevant to their specific needs, consult and collaborate with other professionals providing service to them, and record your activities in a timely manner according to project visit and institutional guidelines.

About the Role
  • Apply the nursing process to assess the six prescribed NFP domains
  • Assist women and their families in establishing realistic goals and measurable outcomes
  • Provide counseling to assist women & their families in attaining targeted goals in areas including prenatal & postpartum care, nutrition, parenting, childcare, family planning, special health problems, housing, continuing education & employment
Requirements

To be considered for this role, you must have a license and current registration to practice as a Registered Professional Nurse in New York State. A Bachelor of Science degree in Nursing from a regionally-accredited college or university is also required, along with a minimum of two years of full-time work experience in maternal/child health, pediatric health or a closely related field.

We offer a competitive salary of $68,000 - $85,000 per year, depending on experience, plus benefits such as health insurance, retirement plan, and paid time off. This position may also be eligible for remote work up to two days per week.
