Job Overview:

As a Gerontology Care Specialist, you will play a vital role in providing high-quality care to residents. Your primary responsibility will be to conduct thorough clinical assessments on a monthly basis, with increased frequency for high-risk patients. You will work closely with an RN to coordinate care, ensuring timely follow-ups, communication, and continuity of care across facilities.

Key Responsibilities:

  • Conduct patient assessments and develop personalized care plans
  • Collaborate with the healthcare team to ensure seamless care transitions
  • Provide primary, preventive, and urgent care on-site, managing chronic and acute needs
  • Prescribe or refill non-narcotic medications after consulting with the resident's primary care provider
  • Facilitate conversations around advanced illness and set personalized goals for each resident
